　　汕頭市關(guān)于在汕外籍人士預(yù)約接種新冠病毒疫苗的告示

　　自2021年5月14日起，汕頭市根據(jù)“知情、自愿、自費(fèi)、風(fēng)險(xiǎn)自擔(dān)”原則，啟動(dòng)我市外籍人士適齡人群接種國(guó)產(chǎn)新冠病毒疫苗（上市階段）工作。

　　我市自愿接種的外籍人士可通過(guò)粵健通微信服務(wù)平臺(tái)在線預(yù)約登記疫苗接種。我市目前使用的是國(guó)產(chǎn)新冠病毒滅活疫苗，全程需要接種兩劑。已參加廣東省社會(huì)保障醫(yī)療保險(xiǎn)的外籍人士，現(xiàn)場(chǎng)出示有效醫(yī)保參保憑證，享受與其他參保中國(guó)公民同等待遇。未參加廣東省社會(huì)保障醫(yī)療保險(xiǎn)的外籍人士，自費(fèi)接種疫苗，費(fèi)用為100元/劑次。

　　預(yù)約方式如下：5月14日起，掃描二維碼（附后）進(jìn)入粵健通微信服務(wù)平臺(tái)的外籍人士新冠疫苗接種服務(wù)頁(yè)面進(jìn)行信息填報(bào)、預(yù)約。預(yù)約成功后在指定時(shí)間、地點(diǎn)，攜帶中華人民共和國(guó)外國(guó)人永久居留身份證或護(hù)照及有效停居留證件、醫(yī)保參保憑證前往。在指定接種點(diǎn)接種前，按程序簽署知情同意書、免責(zé)承諾書等，做好個(gè)人防護(hù)并主動(dòng)告知健康狀況，由專業(yè)人員判定是否符合接種條件。接種14天內(nèi)請(qǐng)密切關(guān)注自身身體情況，如有不適，請(qǐng)及時(shí)就醫(yī)。

　　Notice of COVID-19 Vaccination for Foreign Nationalsin Shantou

　　From May 14,2021, Shantou will start COVID-19 vaccination for foreign nationals subject to local age eligibility restrictions, following the principle of voluntary participation, giving informed consent, self-funded and assuming personal responsibility for risks.

　　Foreign nationals who wish to be vaccinated can make an appointment through the WeChat service platform “粵健通”?(scan the QR code below). China’s domestic inactivated SARS-CoV-2 vaccines will be used, and two doses are required for the entire course. Foreign nationals who have joined Guangdong’s social security and medical insurance scheme share the same treatment as other Chinese citizens, who can be vaccinated free of charge. A valid proof of medical insurance needs to be presented on the vaccination site. For those who have not participated in the social security and medical insurance of Guangdong Province, they will be vaccinated at their own expense---the cost is 100RMB per dose.

　　Appointment process: From May 14, foreign nationals can log in the WeChat service platform “粵健通”?(scan the QR code below) to fill in personal information and make an appointment on the service page for foreign national’s vaccination. After the appointment is successfully made, you should bring your permanent residence ID card, passport or certificate of medical insurance to the designated site at the appointed time for vaccination. Before taking vaccine, you should sign both a form of informed consent and a statement of bearing personal responsibility for all risks associated with vaccination. Please take necessary precautions and proactively inform medical staff of your health condition so that they can determine whether you meet the vaccination requirements or not. If you show any discomfort symptoms within?14 days after vaccination, please seek medical treatment in time.
